Upcoming changes to the NEMT manager

In August of 2022, Veyo, the current manager for non-emergency medical transportation (NEMT) in Wisconsin, was purchased by Medical Transportation Management (MTM), Inc.

Veyo has started their transition to MTM in Wisconsin. The Wisconsin NEMT program has begun using MTM Link, the MTM ride scheduling and mileage reimbursement system. There are no changes to NEMT covered services.

Making a care plan

Each child who is enrolled in Care4Kids has a team that works together to make a care plan that meets the specific needs of the child. The team includes:

  • A health care and outreach coordinator.
  • A child welfare worker.
  • Family members.
  • Foster parents.
  • The child’s doctors.

The team makes sure the care plan continues to meet the child’s needs. They will change the plan if needed.

Picking a doctor

Care4Kids uses Chorus Community Health Plan to provide services. You can pick any doctor or provider in the Plan for your child. Find a doctor online.

Chorus Community Health Plan is accredited by the National Committee for Quality Assurance.

Using your ForwardHealth card

After joining Care4Kids, your child will get a plastic ForwardHealth card in the mail. If they had BadgerCare Plus, Medicaid, or the Family Planning Only Services Program in the past, they won’t get a new card. Use the same care that was used before.

The ForwardHealth card includes your child’s name. It also has a 10-digit identification number and a magnetic stripe. If you need help, the Member Services phone number is on the card. Once your child gets the ForwardHealth card, a parent or foster parent should sign it in the space provided.

Show the child’s ForwardHealth card when visiting the doctor. You also need it when you pick up medicine at the pharmacy.

If your child does not have a ForwardHealth card, or it was lost or stolen, go to your ACCESS account or call ForwardHealth Member Services at 800-362-3002.

Getting services

Care4Kids covers many services, such as health, dental, and mental health care. See a list of key benefits. To find out about coverage for specific services, contact your care coordinator. If a service is not covered, you will have to pay for it.

Note: If your child needs emergency services, go to an in-network provider if possible. Find a list of Care4Kids providers online. If you can’t get to an in-network provider, go to the nearest provider. Tell Care4Kids what happened as soon as you can.
